How to Apply for Florida Licensure

Required Documents

  • FAMA 4-Hour HIV Certificate 
  • FAMA Skincare Diploma with a minimum of 220 hours of education
  • FAMA Transcripts
  • License Application Fee of $75.00
  • Certificate of Eligibility: This document is submitted by FAMA once you have started the application process and received an application number from the DBPR.

Applying for Your Professional License:

  1. Go to the DBPR website at myfloridalicense.com and select Apply for a License
  2. Click on Apply for a License page, and select Cosmetology as the profession.
  3. Under “Getting Started in Cosmetology”, select “Apply for a License”, and click on the “Facial Specialist Registration” (COSMO 1).
  4. You can select apply online (recommended) or print a paper application. You will need to create a DBPR account to start your online application. 
  5. Provide all the required information and check that it is accurate.
  6. Pay the application fee ($75.00).
  7. Attach the Required Documents to your license application.

 

Certificate of Eligibility:

Students must contact FAMA Student Services with their DBPR application number so that we can attach “Section IV” to your application. Please do so by replying to the FAMA email you received with your graduation instructions. 

Finally, once the Board of Cosmetology verifies your documents, including any documents provided by FAMA, you will receive an email with your license attached as a PDF document.


License Renewal:

As a licensed Facial Specialist, you will be required to take a minimum of 16 Hours of Continued Education every two years. Please contact FAMA when the time comes.
